Our consultative service is designed to be individualized to meet the needs of each individual referred to Cornerstone. The process involves conducting a comprehensive Functional Behavior Assessment (FBA), developing of a Behavior Support Plan, and training of the individual and team members on the intervention strategies in the plan.
We focus on spending time getting to know you and understand the presenting problems. Our behavior plans are person-centered and based on goals established with the team for each individual. Our consultants use “out of the box” thinking when developing intervention strategies, though they are always based on the functional assessment and sound Applied Behavior Analysis (ABA) principles. We assist teams to collect data in ways that are least intrusive and revise strategies based on the data collected.

Community Living is an all-inclusive approach to services that emphasizes community inclusion and participation to improve individuals' overall quality of life. This service provides individuals with more options in staffing while ensuring they receive the necessary support to meet their daily needs. Access to 24-hour on-call support is crucial to help individuals develop, maintain, or enhance their skills, all while ensuring their personal safety. Community Living focuses on various areas, including self-care, health maintenance, wellness activities, meal preparation, decision making, home and money management, communication, mobility, transportation, relationship development, socialization, personal adjustment, community participation, and utilization of community resources.
Our Life Sharing service is designed to provide support, resources, and financial assistance to Life Sharing Providers, allowing them to continue providing care for their loved ones in the comfort of their family home. This option allows individuals to live in a minimally restrictive environment without interrupting their daily routine, while still receiving support from a trusted family member as they develop new skills and grow.
Our housing transition service helps individuals with disabilities become successful tenants in their privately owned, rented, or leased homes by providing pre-tenancy and housing sustaining support.
The concepts and philosophy of person-centered planning are incorporated throughout the development and implementation of the individuals housing plans.
The housing team will work diligently to prioritize personal needs and proactively address potential barriers. Individuals will be assisted through every step of the process—from housing search up to, and including, move-in day.
